- Supports service providers, link groups or systems that provide synergistic support, and implements advocacy strategies to address unmet needs
- Supports clients with mental health and substance abuse challenge to engage with mental health and substance use treatments
- Develops health education curriculum using participatory methods and learners' experiences to guide the content of the curriculum
- Assesses health education needs in the communities
- Conducts, reports, and communicates results and implications of basic screening tests (height, weight, blood pressure, glucose level, etc.)
- Visits homes to check on individuals with specific health conditions
- Serves as a liaison between medical and social services and the community, advocating for change and improving access to quality of care
- Provides direct services, such as informal counseling, social support, care coordination and health screenings
- Helps clients understand their health conditions and develops strategies to improve their health and well being
- Develops goals, objectives, and strategies for programs
- Builds relationships and learns about the lives of clients, the resources and needs of their communities, and the barriers to optimum health
- Connects individuals to health and human service resources, such as health insurance, food, housing and other health information
- Conducts home visits or meets with individuals outside of the clinical setting to provide services
- Designs, implements and interprets individual and community-level assessments (e.g. home environmental assessment; community assets and challenges)
- Facilitates, obtains or coordinates transportation to services and helps ameliorate other barriers to services
- Communicates with other workers and agency professionals regarding client and community needs and concerns
- Acts as frontline outreach advocate and liaison service provider linking individuals and communities
- Tracks, reports, shares and utilizes data for in-patient care and service delivery
- Writes reports on clients, outreach, and health education activities and related project
- Improves the value and quality of cultural competence in human service care delivery
- Applies professional skills, ethics, professional boundaries, and cultural humility, in working with diverse clients and communities
- Provides active listening, support, and coaching to patients on life changes that support health and wellness
- Attends to behavioral health diagnosis and treatment issues for clients
- Delivers health information using culturally appropriate terms and concepts
